临床疗效比较的一项荟萃分析——以“近端胃切除双通道吻合与全胃切除 Roux-en-Y 吻合”为例

摘要：目的：对比分析近端胃切除双通道吻合与全胃切除 Roux-en-Y 吻合的临床疗效。 方法：从 PubMed、E mbase、Cochrane Library、Cnki、万方数据库、维普中文期刊数据库检索关于近端胃癌切除双通道吻合及全胃 切除 Roux-en-Y 吻合的研究文献，通过筛选文献、提取数据，使用 Stata12.0 软件对提取的数据进行 Meta 分析。 结果：Meta 分析结果提示双通道组患者术后总体并发症发生率低于 Roux-en-Y 组（OR=0.599，95%CI：0.39 8-0.901，P=0.014＜0.005)，且 1 年后血红蛋白量水平有所改善 MD=0.705，95%CI：0.514-0.896，P=0.000＜0. 005);两组患者食管反流发生率无明显差异（OR=0.612，95%CI：0.264-1.423，P=0.254＞0.005）。结论：与 Ro ux-en-Y 吻合术相比，双通道吻合术不仅能减少术后并发症的发生，而且在改善患者生活质量方面具有显著优势， 是一种安全有效的消化道重建方法。
